Preparing for your first appointment
When you come to our office, we want you to know that we greatly appreciate your trust and confidence. We will treat you promptly and professionally with dignity and respect. Our doctors are always cognizant of their appointment times. If unforeseen delays should develop, we will keep you informed. Our goal is to make your appointment with us both pleasant and meaningful.
In preparation for your first appointment, there are a few things you can do to help us. Please collect any pertinent medical records, including x-rays and MRIs. If you have had imaging studies, please try to bring the actual films and not just the reports. These will be helpful for us to review. Please also download and print the followingand complete it prior to your visit. Alternatively, plan to come fifteen minutes prior to your appointment, at which time you can complete the paper form in our office.
We have gowns in our office for you to use during the physical examination. However, if you prefer, please feel free to bring comfortable gym clothes (such as shorts and t-shirt). We look forward to seeing you.
